DEA License Application & Renewal
Residents and fellows who write prescriptions are required to register for an individual DEA license. Licenses may be obtained on a fee-exempt basis while in training in a UW GME training program; however, such licenses are restricted to activities within the scope of the training program (including activities at UW Medicine sites and other affiliated training sites). Application (Form 224) for new license registrations are to be submitted online via the DEA website. Residents and fellows must hold a current Washington state medical license at the time their DEA license application is submitted. New applications are processed in 4-6 weeks. For the fee-exempt license, the address must be the address of the fee-exempt institution (University of Washington) and not a home address. Please use the Psychiatry Residency Office address (1959 NE Pacific St., Room BB1627, Box 356560, Seattle, WA 98195-6560). The Fee Exempt Certifier is Athena S. Wong at 206-543-6577.
Renewal notices are mailed by the DEA Office approximately 60 days prior to the expiration date. Residents and fellows may complete the standard renewal form or renew (Form 224a) their license online. Renewals are processed in approximately 4 weeks.
