Renewing a Full Osteopathic License in the State of Washington (DO)

The following are instructions and forms for renewing a full osteopathic license. If you are renewing a limited osteopathic license, limited or full medical license, or are applying for a license, please go to the licensing page and select the appropriate option.

Full osteopathic licenses expire after two years on your birthdate. The process for renewing a full license is based on a license renewal reminder sent by the Board of Osteopathic Medicine and Surgery. The Board will detail what documentation they require for your renewal and whether any documentation of continuing education credit is required. Remember that serving as a resident is sufficient to fufill all continuing education requirements. It is the resident's responsibility to begin the renewal process as soon as possible after receiving your renewal reminder. If you do not receive your renewal reminder 8 weeks prior to the expiration of your license, please contact the Board. Residents without a valid license on file will be pulled from clinical services and may not receive credit for rotations.

Step 1: Submit a check for a two-year renewal (be sure to write your license number in the memo field of your check) for $475 to Board at the following address:

Step 2: Follow up. If you have not received your renewed license after 8 weeks, please call the Board at (360) 236-4700 to determine the status of your renewal.

Step 3: Provide a copy of your renewed license to the residency office.
