Applying for a Limited Medical License in the State
of Washington (MD)

Consistent with the medical staff bylaws of UW School of
Medicine’s participating institutions, all residents and fellows must
hold an active medical license while training in a UW graduate medical
education program. The following are instructions and forms for applying
for a limited medical license. If you are applying for a full medical
license, osteopathic license or are renewing a license, please go to the
licensing page and select the appropriate
option.
The Washington State Medical Quality Assurance Commission often
takes several weeks to process medical license applications and renewals.
In order to minimize the time that it will take to process your application
you must ensure that you precisely follow the instructions
provided by the Commission. As the instructions provided by the Commission
can often be confusing, the following summary has been provided to assist
you in your application for licensure.
Step 1: Download and print the Residency
Certification form. Fill this form out and submit this form to your
program director to certify your status as a resident.
Step 2: Download and print the Medical
School Transcripts Request form. Fill this form out completely and
send directly to your medical school registrar. Medical school transcripts
must show the date that you have received your degree and must
be sent directly from the institution. It is your responsibility
to ensure that any information required by your medical school registrar
is included, as well as any fees for printing official transcripts.
Step 3: Download and print the Application
for Limited Medical License. Fill out each section completely and
accurately. Attach a current photograph according to the instructions
to the application. Send the application, attached photograph and licensing
fee directly to the Medical Quality Assurance Commission at the address
listed on the form. The application, photograph and check for your fee
of $235 are the only forms sent directly to the Commission.
The following forms should be submitted if applicable:
- American
Medical Association (AMA) Profile: required if you have completed
post-graduate medical training or have been licensed in another state
or province. If applicable, fill our this form and send directly to
the American Medical Association at the address listed on the form.
If you are not a member of the American Medical Association, you must
include a check to pay a fee to have the AMA process your profile.
- Federation of
State Medical Boards (FSMB) Data Bank Clearance: required if you
have completed post-graduate medical training or have been licensed
in another state or province. If applicable, fill this form out completely
and send directly to the FSMB at the address listed on the form.
- Hospital
Privilege Verification: required if you have been granted hospital
privileges within the last five years. If this is the case, download
and print copies of this form for each hospital where you were
granted privileges. Fill out the appropriate sections of each form
and send directly to the hospitals where privileges were granted.
- Post Graduate
Training Verification: required if you have completed post-graduate
training in another state or province. If applicable, fill in the
appropriate sections of this form and send directly to the post-graduate
training director of that program.
- State Licensure
Verification: required if you have ever had a medical license
in another state or province. If this is the case, download and print
copies of this form for each state where you have held a medical
license. Fill out the appropriate sections of each form and send directly
to the state licensing board of each state where you held a license.
Follow up. If you have not received your renewed license after 6 weeks,
please call the Commission to determine the status of your renewal.
