Renewing a Limited Osteopathic License in the State of Washington (DO)

The following are instructions and forms for renewing a limited osteopathic license. If you are renewing a full osteopathic license, limited or full medical license, or are applying for a license, please go to the licensing page and select the appropriate option.

Limited osteopathic licenses expire one year after your anniversary date in the residency program. Due to limited staffing, the Board of Osteopathic Medicine and Surgery does not provide notice that a limited license is expiring. It is the resident's responsibility to begin the renewal process at least 8 weeks prior to the expiration of their license. Residents without a valid license on file will be pulled from clinical services and may not receive credit for rotations.

Step 1: Download and print the Residency Certification form. Fill this form out and submit this form to the program director to certify your status as a resident. The residency office will forward this directly to the Board.

Step 2: Submit a check (be sure to write your license number in the memo field of your check) for $250 to Board at the following address:

Step 3: Follow up. If you have not received your renewed license after 8 weeks, please call the Board at (360) 236-4700 to determine the status of your renewal.

Step 4: Provide a copy of your renewed license to the residency office.
