Renewing a Limited Medical License in the State of Washington (MD)

The following are instructions and forms for renewing a limited medical license. If you are renewing a full medical license, osteopathic license or are applying for a license, please go to the licensing page and select the appropriate option.

Limited medical licenses expire one year after your anniversary date in the residency program. Due to limited staffing, the Washington State Medical Quality Assurance Commission does not provide notice that a limited license is expiring. It is the resident's responsibility to begin the renewal process at least 8 weeks prior to the expiration of their license. Residents without a valid license on file will be pulled from clinical services and may not receive credit for rotations.

Step 1: Download and print the Residency Certification form. Fill this form out and submit this form to the program director to certify your status as a resident. The residency office will forward this directly to the Commission.

Step 2: Submit a check (be sure to write your license number in the memo field of your check) for $235 to Washington Department of Health at the following address:

Step 3: Follow up. If you have not received your renewed license after 8 weeks, please call the Commission to determine the status of your renewal.
